Ketamine Coaching Client Agreement

This agreement is made between Being True To You ("BTTY") and Your Name* ("the Client").

1. Services Provided

Transformational & Integration Coaching:
Being True To You offers virtual coaching services to support clients before, during, and after transformational experiences, such as ketamine treatment. Our services include but are not limited to:

Individual Coaching Sessions: Delivered via phone or video calls.
Self-Study Materials: Access to downloadable workbooks and resources.
Character & Skills Development: Training and reflective tools to build resilience.

Purpose:
Our coaching aims to mentally and physically prepare clients for transformational experiences, and assist in integrating insights post-experience. We guide clients in aligning with their true selves, setting and achieving personal goals, overcoming obstacles, and enhancing their personal and professional lives.

2. Client's Autonomy

Clients are empowered to choose the focus of their coaching sessions. They are encouraged to make informed decisions, think independently, and determine their path without pressure or coercion.

3. Disclaimers

Not a Substitute for Professional Therapy:
BTTY coaching is a supplementary service and does not replace medical treatment or counseling. We do not diagnose, treat, or evaluate mental health conditions, physical diseases, or addiction issues. Clients should seek professional help as needed.
Limitations of Responsibility:
BTTY is not responsible for the client's choice of treatments, medications, or safety during the coaching process. We support clients around their transformational experiences but are not liable for those experiences or any external programs or clinics they engage with.
No Medical Advice:
BTTY does not endorse, recommend, or advise on the use of ketamine or other psychedelic treatments. Clients are solely responsible for their health decisions, including the use of alternative medicines.
No Guarantees:
BTTY does not guarantee outcomes such as healing, recovery, or success. The client's progress depends on their commitment and actions.

4. Informed Consent

By initialing the below, the Client acknowledges and agrees to the following:

I understand that BTTY provides integration coaching to prepare me for and help me process therapeutic experiences like ketamine treatment.I am aware that coaching focuses on my goals and holds me accountable for achieving them.I understand that to achieve my goals I will have to make changes within my own mindset, character, relationships, lifestyle, and way of being.I understand that coaching may reveal emotional challenges, which are a natural part of growth.I acknowledge that coaching is not professional counseling or psychotherapy, and my coach is not a licensed therapist.I understand that while coaching is confidential, certain disclosures (e.g., harm to myself or others) may require a breach of confidentiality.I am responsible for all medical and therapeutic decisions, including the use of any treatments or medications.

5. Terms & Conditions

Commitment:
Upon signing, the Client commits to completing the agreed number of sessions. Missed sessions not canceled with at least 2 hours notice may incur full charges.
Payment & Refund Policy:
All payments must be made before services are rendered. Refunds are not available for processed payments. If a Client discontinues payments or terminates coaching early, they forfeit all payments made
Additional Services:
Additional coaching sessions may be purchased at any time through Being True To You.
Clinic Communications
BTTY reserves the right to communicate coaching session progress and updates with the partner clinic. This includes and is not limited to communicating the number of coaching sessions completed, the number of coaching sessions remaining, the cessation of coaching and the continuation of coaching.

6. Payment & Duration

The Client has selected and paid for a coaching package tailored to their needs, with the option to purchase additional sessions through Being True To You under this agreement. The agreement begins on the date of the first payment and remains valid throughout the coaching period.
